If you are receiving an income-tested benefit from Work and Income and you are receiving family support with it,  you are not required to fill in this form.
What you will need

  • IRD number
  • Bank account number
  • The amount of income BEFORE TAX (this may show as "gross" income on your payslip).
  • The amount of income for the WHOLE year (1 April 2008 to 31 March 2009).
  • Your partner's IRD number, full names and details of income earnt.
  • The details of the children in your care - full names, date of birth, date the child started living with you (if different from date of birth) and the children's IRD numbers.
  • An ACCURATE income amount so we don't underpay or overpay you.
